  1. このフォームはアルセアタワー19階会議室でご利用になられたペットボトル飲料の精算フォームです。
  2. お飲みになったペットボトルの本数、ご利用日、部課名、担当者お名前、メールアドレス、相手先の会社名などをご入力の上、送信ください。入力された内容は担当者様に自動送信されます。フォームの送信は会議前の飲料持ち出し後、必ず実施ください。
  3. アルセアタワー19階会議室用のペットボトル飲料は、アルセアタワー会議室利用者専用となりますので、他のビル・会議室への持出しは厳禁です。また個人でのご利用もできません。
  4. １ケース（２４本）以上ご利用される場合は予めラインへご連絡ください。
  5. 会議が終わりましたら、飲み終えたペットボトルはリサイクルボックスに入れてください。また飲み残しがある場合は給湯室へ捨てた上で、リサイクルボックスに入れてください。
